APPOINTMENT INFORMATION
Office hours are 7:30 AM to 5:00 PM, Monday through Friday. Late hours may be available by appointment only.
We are also available from 8:00 AM to 12:00 PM on Saturday by appointment only.
If you are calling for a well-child visit or routine medication refill, we ask that you call after 10:00 AM to keep the early morning phone lines open for sick patients. We make every effort to see sick children in a timely manner.
AFTER-HOURS EMERGENCIES
If your child has
a life-threatening emergency, call 911.
Phone calls for urgent after-hours concerns are handled
by our on-call providers who are available after hours and on weekends and holidays. Urgent illnesses
and injuries may be seen in the office on weekend nights and Saturday mornings if the on-call provider
is in the clinic. The provider may choose to see after-hours patients with urgent needs in the
hospital or emergency room.
WELL CHILD VISITS
Please call to schedule well child visits 30 to 60 days in advance. Well
child visits are recommended beginning at 2 days, between 8-14 days, and then at 2, 4, 6, 9, 12, 18
and 24 months of age.
After age 2, visits are recommended yearly with a pre-kindergarten visit at age 5, which includes vision, hearing screening and immunization updates.
Children and teens playing school sports require a yearly physical. You are responsible for bringing any forms or paperwork with you for completion. Please check with your insurance company for the schedule in which well visits are reimbursed.
MEDICATION REFILLS
For ADD/ADHD and routine medication
refills,
our medication refill form can be printed from this website or picked up at our office and brought
back or faxed back to the office 5 to 7 days prior to the prescription running out.
CANCELLATIONS
If you need to cancel an
appointment, we ask that you give us 24-hour notice. If you do not show up for an appointment
and fail to give notice, future limitations for scheduling will apply.
